I was a Coordinator within the Looked After Children (LAC) team. All children who are placed into Local Authority care have to receive an initial LAC medical (statutory guidelines dictates this) and I was responsible for liaising with Social Workers, Foster Carers, Key Workers and City Council administrators to obtain the relevant information in relation to each child and ensuring the relevant consents have been obtained. I collated the accurate information in relation to their demographics, Care plans and placement information to upload onto the NHS Care notes system. For Safeguarding purposes and sharing of relevant information I also ensured any other authorities were updated, as appropriate, such as local CCG's or neighbouring trusts.
I liaised with the Pediatrician’s for their availability and proceeded to arrange their clinics. This involved booking clinic rooms through an electronic system, preparing the relevant paperwork, informing the Carers and Clinicians of the appointment details and recording the appointment in the Care notes system. I updated and maintained KPI spreadsheets (in Excel) so all of the appointments were conducted as a tracker system. I used this data to forecast the number of children who will need their medicals on a rolling two months in advance.
Furthermore, I liaised with the LAC nurses and following weekly allocations meetings with them I arranged for the Looked After Children to be appointed for their annual health review. The LAC could be placed throughout the country as well as in Coventry. This involved managing Nurses electronic diaries and liaising with the Foster Carers, other trusts and Social Workers to agree upon an appointment. I also organised the typing of the finalised reports, and created a tracking system to ensure that no statutory dates were breached.
